STEP 4

Define what to do to achieve your target outcomes

Rationale

Interventions will have to be adapted to your reality

Following the gaps in care evaluation, and the identification of obstacles and your strengths, you have to prioritize which targets you want to focus on. And now, you have to select the best interventions or adaptations/readjustments to existing interventions/programs/clinical pathways (implementation strategies).

Local practitioners are essential

Remember that the interventions have to be adapted to the reality of your organization and accepted by the practitioners/managers. The "local" practitioners are essential to achieving success in the implementation phase in our specific setting and for our specific population, as well as vis-Ó-vis the managers, so as to win their support.

YOU NOW PROCEED TO THE IMPLEMENTATION PHASE

A) Understand the barriers/obstacles in your environment and define potential solutions

B) Select suitable interventions

C) Define your intervention plan

By identifying obstacles to adherence, and including healthcare professional perspectives on potential barriers and their solutions to bridge the gaps in care, a more successful intervention can be achieved.
